Senior Programs
Elderly Victim Assistance Program
**Serves ages 60 and over. Hours 9:00 a.m. – 5:00 p.m., Monday – Friday. Provides limited financial loss, property damage and personal injury to senior and indigent victims of crime. Check and Respond Everyday (CARE)
**CARE is a twenty-four hour computerized phone service that will call the homes of elderly and handicapped up to three times a day to check up on them. After the resident answers the call, the system will identify itself and prompt the resident to press 1 if everything is fine and 2 if an alert number needs to be notified. This service is offered free of charge to residents of Chesapeake. Hospice ServicesMeals On Wheels
**Meals on Wheels of Chesapeake provides nourishing meals to residents of Chesapeake who are unable to provide, obtain or prepare their own meal due to disability, age or illness. Meals on Wheels delivers two meals per day (one hot meal, one cold meal) Monday through Friday. The cost is $3.40 per day for the two meals. File of LifeChesapeake Sheriff's Office **The File of Life is a medical history card that is distributed by TRIAD. The card is placed on the senior's refrigerator and will notify the Fire Department and Emergency Medical Technicians of medication, medical insurance and who to contact in case of emergency. This service is provided free of charge. Special Care and Needs Programs (SCAN)
**SCAN has participants supply Chesapeake Emergency Services with medical information. In case of emergency, this medical information is sent by computer to fire personnel responding. SCAN forms are available at all Fire Departments and Fire Headquarters. The Pharmacy Connection
The Pharmacy Connection links people with pharmaceutical needs with the pharmaceutical companies that provide free prescription drugs to seniors on fixed incomes. Senior Services of Southeastern Virginia acts as the contact for this important linkage by completing application(s) for the pharmaceutical company. Virginia Poison Center
Virginia Poison Center is a twenty-four hour hotline that offers advice and referral services in case of accidental poisoning. Virginia Insurance Counseling and Assistance Program V.I.C.A.P.
VICAP was established to educate, counsel, and assist senior citizens on medical benefit programs, long-term care insurance and problems with Medicare, Medicaid and medical bill paying. This program was established in cooperation with the Virginia Department on Aging and the State Corporation Commission Bureau of Insurance. Call 222-4512 to schedule an appointment with a counselor or for further assistance. Real Estate Tax Relief for Senior and Disabled Citizens
Defrays taxes for eligible senior and disabled homeowners. The exemption is granted on an annual basis for those applicants that qualify. Applicants must apply for the program each taxable year by May 15th. To schedule an appointment, contact the Commissioner of Revenue’s office at 382-6698 or 382-6734. Adult Protective Services
Adult Protective Services is mandated by the Code of Virginia. These services must be provided, regardless of income, to persons 60 years and older and incapacitated adults over the age of 18, who are reported to be abused, neglected and/or exploited. The Chesapeake Department of Human Services-Division of Social Services receives reports, conducts investigations to determine the need for protective services and provides or arranges for services needed. Reports may be made Monday through Friday, 8:00 a.m. to 5:00 p.m. by calling 382-2000 and asking for Adult Services. Adult Services are provided to persons with disabilities 18 years of age and older, to persons 60 years of age and older, and to their families when appropriate. Adult services include case management and other assistance to help adults remain in their own homes, i.e.home-based care, transportation, adult day care, and others. Adults who need placement outside of their homes may receive a pre-admission screening to determine the need for nursing facility placement or may receive an assessment to determine the need for placement in an adult care residence. Companion Services
Companion Services may be provided to an adult who is unable to care for him/herself without assistance. Eligibility for companion services is based on companion assessment and income eligibility. Currently, the financial eligibility guideline for one person is a maximum of $1157 gross monthly income. Once the adult is approved for services, an agency approved companion provider can assist with light housekeeping, meal preparation, shopping, laundry, medication management, bathing and dressing. This program is not intended to provide total care. The number of approved hours per week is based on the individual's need and the agency's guidelines. Adult Foster/Family Homes
Adult Foster/Family Homes are private homes which provide room and board, supervision and special services to an adult who has a physical, mental and/or emotional problem(s). The adult must be incapable of independent living. The homes and providers are certified by the Chesapeake Department of Human Services-Division of Social Services prior to placement and monitored following placement of an agency customer. The provider may assist the customer with daily living skills such as bathing, dressing, grooming, medication management and other personal care needs as required. The Adult foster/Family Homes accept no more than three adults. This family-oriented program allows for more individualized attention and enables the customer to remain in the community. Auxiliary Grants
An Auxiliary Grant (AG) is a financial payment, which supplements income for persons who live in an adult care residence (ACR) or adult family home. Eligibility for the program is determined by the local social services agency where the individual resided prior to admission to the ACR or adult family home. For more information, ask for anyone in the Long Term Care Intake Unit. The Hampton Roads Long-Term Ombudsman Program
A joint project sponsored by Senior Services of Southeastern Virginia and The Peninsula Agency on Aging serving all of Hampton Roads as an advocate for older adults and their families. The Ombudsman Program resolves complaints and concerns by investigating issues involving quality of care, quality of life and residents rights in adult care facilities, nursing facilities and home health services. Their goal is to investigate the problem and provide assistance along with education and information on long term care services. Senior Identification Cards
Identification cards are made for senior citizens 55 years and older on the third Wednesday of each month from 9:00 a.m. to 12:00 noon at the Chesapeake Senior Center. Registration requires two forms of ID. The Chesapeake Sheriff's Office and Parks and Recreation Department offer this service free of charge. ComfortCare Home Health Services
ComfortCare Home Health Services, a part of the Chesapeake Health family, has been offering the same quality care at home that people have come to expect from our acute care setting since 1983. As an extension of the hospital, our licensed and bonded staff comprised of nurses, occupational and physical therapists, speech language pathologists, dieticians, home health aids and social workers provide medical attention for patients requiring care in the home setting. Services are provided in the City of Chesapeake and other surrounding areas including Norfolk, Virginia Beach, Portsmouth, and Suffolk. Specific service areas include Skilled Nursing, Diabetic Intervention, IV therapy, Rehabilitation, Psychiatric Services as well as Home Health Assistance services. Philanthropic Advisory Service develops and distributes information of charitable organizations that solicit nationally by evaluating if the charities meet the 23 standards for charitable solicitations under the Council of Better Business Bureaus, Inc. Detailed reports summarizing the evaluations are distributed to inquirers. Substance Abuse Services
Provides individual and family counseling, as well as substance abuse assessments, evaluation, urinalysis testing, training and education to Chesapeake citizens, who are experiencing problems related to the abuse of chemical substances. Services inclusive of medical, social, methadone detoxification, shelter placements and primary care programming are also available.

Lifeline
Lifeline is the oldest and largest personal emergency response service in the industry. Over the past 20 years, million of subscribers have relied on Lifeline to enable them to remain independent in their own homes. By activating a Personal Help Button, worn as a pendant or on the wrist, a person can automatically dial the Lifeline Response Center and receive appropriate assistance for the situation; an ambulance, fire department, police or simply a loved one to come by and check on you. Lifeline subscribers pay a low, one time sign-on fee, plus a monthly monitoring fee. The Elder Care Locator is a nationwide, directory assistance service designed to help older persons and caregivers locate local support resources for aging Americans. This is a public service of the Administration on Aging, U. S. Department of Health and Human Services and is administered by the National Association of Area Agencies on Aging and the National Association of State Units on Aging. Personal Emergency Response System—Health Watch
Health Watch is the exclusive Personal Emergency Response System provided by Bon Secours Private Care. This system may provide the vital link that allows clients to live independently at home and feel safe and secure regardless of their age or physical condition. Help is just a push-of-the-button away.
Health Watch consists of 3 components: 1) personal help button or transmitter (necklace, bracelet, belt -clip); 2) help console which plugs into telephone line; and 3) response center. When a subscriber needs help, he or she follows the ABC's" A) push the help button on personal transmitter or console; B) the help console signals the Health Watch Response Center; C) a trained operator attempts to talk to the subscriber through the help console. The operators know the subscriber's situation and will contact the designated responders. If the responders are not available to assist, or the subscriber is not able to reply, EMS is dispatched to the home. Health Watch is available throughout Hampton Roads. Subscribers pay a one-time installation fee and monthly service charge. Limited grant money is available for indigent clients. Clients and loved ones, who use the service agree that the "peace-of-mind" and "sense-of-security" Health Watch offers is well worth the cost. Bon Secours Home Care
Bon Secours Home care is a Medicare/Medicaid certified, JCAHO accredited full service home care agency. We are celebrating our 20th year providing "Good Help To Those In Need…..A Home!" Our services include: 1) comprehensive adult and pediatric skilled nursing, with specialists in infusion therapy, maternal-child and psychiatric nursing: 2) physical therapy, speech-language pathology and occupational therapy for rehabilitative needs; and 3) medical social workers and home health aides, when covered by insurance. Skilled nurse and home health aide visits are made on an intermittent basis. The patient must need a medically necessary skilled service, which is ordered by a physician. Medicare and some other insurers require that the patient be homebound. In hospitals or nursing care facilities, the discharge planner consults with the physician and obtains an order for home health services. The discharge planner then contacts our nurse liaison to assist the patient in transitioning from hospital to home. Referrals may also be called in from physician offices. Home health services are provided in Chesapeake, Norfolk, Virginia Beach, Portsmouth, Suffolk, parts of Isle of Wight, and Smithfield. Bon Secours Hospice
Bon Secours Hospice has the distinction of being the fist hospice in Hampton Roads. We are committed to providing compassionate care to terminally ill patients and their families. When curative treatment is no longer an option, receiving compassion, sympathy and comfort measures are important. Our goal is to meet physical, emotional and spiritual needs so that whatever time remains is valuable to the patient and loved ones. Patients are referred by their treating physician, who continues to manage care. The hospice team includes nurses, social workers, nursing assistants, pastoral care and volunteers. Bon Secours Private Care is a Medicaid-certified agency. Private Care is a nursing service, that provides one-on-one care from nurse's aides with case supervision by a registered nurse. Services can be provided to patients in their home or hospital room. Our program is valuable to patients, who are recovering from an illness or injury as well as benefiting those who need help with daily living activities. Our services include the following: nurse's aide to assist with bathing and dressing; preparation of meals based on dietary needs for patients unable to cook; light housekeeping while caring for patient, to ensure living area is clean; and assistance with grocery shopping and other errands as needed. Most private care is contracted by and paid for by the client. Occasionally private insurance companies, local social services or Medicaid will cover private care. |
